DOT Physical Services
All of our providers at Premier Primary Care Medicine have been certified by the FMCSA to conduct DOT physical examinations. The DOT physical is an important part of receiving or maintaining your CDL license. During the exam the provider will review your medical history and conduct a physical exam to determine that you are physically, mentally and emotionally fit to be driving a commercial vehicle. DOT physical services are offered at all our office locations.
What to Expect
During your visit a medical history will be taken, your vision will be tested, and a physical exam will be performed. A urine collection will also be taken for testing so please come with a full bladder. For certain medical conditions, we may refer you for further clarification on your medical condition.
Fees
The DOT physical will not be billed to your insurance. You are responsible for the DOT physical fee. Please contact our office for the current price. There will be no refunds issued if you fail the exam or if your exam gets put into determination pending status or if you leave before the exam is completed.
What to Bring
To ensure that your appointment goes smoothly, please bring the following items:
1. Your driver’s license
2. To speed up the appointment, please complete our office New Patient Registration form and also page one and two of the DOT Medical Examination Form (driver’s portion) to save time at the clinic.

3. Current medications. Bring all your pill bottles. If that’s not possible bring a list of your meds with the dosages and frequency and the name of the prescribing physician.
4. Medical records. Please bring any medical records you have from your primary care doctor and specialist physicians.
5. Eyewear or Hearing aids. Please bring your glasses / contact lenses and hearing aids if you wear them.
Important Reminders
1. If you take high blood pressure medications please take your medications as usual before coming for your exam.
2. If you have diabetes:
a. Bring your home blood sugar log report. The log cannot be hand written. It must be a report generated by your glucose machine.
b. Bring the most recent results of a lab test called a Hemoglobin A1C (HgAIC) and any other lab results you have.
c. If you are using insulin, we cannot perform your exam until you have Form MCSA-5870 completed prior to your appointment. You must have the treating physician (PCP or Endocrinologist) fill out Form MCSA-5870 within 45 days of the DOT appointment date. Here is the link for Form MCSA-5870
3. Drivers who have nighttime sleep disturbance (sleep apnea) and use a CPAP machine
a. Bring a reading from your machine documenting your proper use of the machine (“Compliance Report”); Bring at least 90 days of data, but if you can get all the data from the past year, that would be best.
b. We may require a form for your treating physician to fill out.
4. Drivers who have heart-related issues, (including history of stent, valve replacement, pacemaker, open-heart surgery, cardiac bypass surgery, or heart attack)
a. Please bring all of your medical records pertaining to your heart condition.
b. If possible also bring the results of a recent stress test, ECHOcardiogram, or other testing completed within the past 1-2 years
5. Drivers who have suffered a stroke, a brain tumor, seizure disorder, or bleeding in the brain: Please bring all of your medical records pertaining to your neurologic condition.
6. Drivers who have experienced the permanent loss of use in an arm or a leg
a. Bring an overview from your physician of the injury and if you have any work restrictions due to the injury
b. You may need a Skilled Performance Examination in order to qualify for your DOT card
7. Drivers who are taking any medications that may cause sedation or sleepiness or controlled substances (includes narcotics, sleeping pills, anxiety medication, ADHD medication)
a. Please bring medical records from your treating physician regarding your condition and medication names, doses, and instructions.
b. Some medications may disqualify you from passing the DOT exam.
8. Drivers who are taking the blood thinner Coumadin (Warfarin)
a. Bring your lab report for PT/INR results for the past few months.
b. We may require a form for your treating physician to fill out.
9. Seizure Disorder (Epilepsy)
a. If you have seizure disorder or epilepsy please bring all medical records pertaining to your seizures.
b. We will require a form to be filled out by your treating physician.
c. You may require a referral to FMCSA for an FMCSA Seizure Exemption form evaluation before you can be certified
10. Psychiatric History (Depression, Anxiety, Bipolar, PTSD, Schizophrenia, etc)
a. Please bring records from your treating physician for review
b. Please bring all current meds you are on with dosing information
c. You may be required to have a form filled out by your treating physician.
11. If you have been diagnosed with Monocular vision in the past you must have your eye doctor fill out Form MCSA–5871 prior to your appointment with us. Form MCSA-5871 must be completed within 45 days of the DOT appointment date. Here is the link to Form MCSA-5871
